Ready, Set, Go: Finding Positive Solutions to Support a Successful Landing in the Adult World

December 2023

The Child Health Hub in Transition to Adult Healthcare hosted the 2nd Transition to Adulthood Pop-Up event on December 2-3, 2023, at the beautiful Hyatt Regency in downtown Vancouver. The event brought together youth, families, trainees, researchers, clinicians, and policy/decision makers to celebrate the best of what they were learning, and doing, to help young people to develop fully, even when they are challenged by health conditions.

Inspired by the Children’s Healthcare Canada strategy, the vision for the inaugural Canadian Transition to Adulthood Pop-Up Event held January 26 and 27, 2021, was centred on how we build bridges to promote the best possible health outcomes for youth and families in Canada. We  celebrated the best of what we are learning, and doing, to help young people to develop fully, even when they are challenged by health conditions.
Children's Healthcare Canada and our Health Hub in Transition with support from McMaster Children's Hospital and CanChild hosted over 200 delegates at this virtual event.

The “Meet the Experts on Transition” series is hosted by the Health Hub in Transition. The aim of the series is to share different perspectives on transition – from youth, parents, researchers, to healthcare providers.

The Provincial Council for Maternal and Child Health (PCMCH) and Ontario Health are hosting “Changing the Story: Improving Transitions from Youth to Adult Health Care,” a webinar for those involved in the delivery of care to young people who will transition out of youth-oriented health care services and into adult health care services, including health care providers, managers, administrative leaders, and young people and their parents and caregivers.
 
 Co-hosted by Sanober Diaz, Executive Director of PCMCH, Dr. Kristin Cleverley of the Centre for Addiction and Mental Health, and Dr. Alene Toulany of the Hospital for Sick Children, this webinar will feature panelists with lived experience and clinical experience sharing their insights on how implementing the Transitions From Youth to Adult Health Care Services quality standard will improve the transition process for young people, their parents and caregivers, and the health care providers who support them.
