Environmental Scan

Youth and young adults with special health care needs often experience challenges when transitioning from pediatric to adult care as a result of the numerous gaps and barriers that exist across health and social care services. While care providers struggle to support this population, young adults suffer the consequences of transitions that are not handled properly. When transitions are not properly supported there are a number of negative outcomes, including: fragmented care, increased emergency department visits, poor access to care, decreases in health outcomes, as well as negative experiences for patients, caregivers, and care providers. The Transition Hub is proposing to conduct an environmental scan to map the current state of programs, services, and resources offered to support the transition from pediatric to adult health care across Canada.
